Volunteer Application and Criminal Background Authorization and Release
All new troop cookie managers are required to complete a Volunteer Application and Criminal Background Authorization and Release (GSSJC F- 39). If you have not yet completed this form, bring the following information with you when you come to your training:
- Troop number and Service Unit name in which you are registered (if unknown, ask your troop leader)
- Driver’s License and Social Security numbers
- Names of two non-related people who will serve as references
- Complete addresses, phone numbers and e-mail addresses for those references.
